Dear Patients,

It is with mixed emotions that I announce my retirement from Lockhart Primary Care,

effective May 1, 2025. It has been a great pleasure providing for your health care

needs over the years, and it is not easy for me to step away.

Rebecca will be transitioning to practice with the Christus Trinity Clinic at
1320 Wonder World Dr, Suite 101 in San Marcos. If you choose to continue care

with her, your follow up appointments may be seamlessly transitioned to the

San Marcos clinic by calling 512-396-3911 to
schedule.

You may also elect to transfer your records to any health care provider of your choosing.
Checking with your insurance for a provider in network may be a good place to start. Please
remember that it will be important to follow up with either Rebecca or a new health care provider
of your choosing as per recommendations at your most recent office visit. Any future diagnostic
tests that have been ordered (i.e. labs, mammograms, etc.) will be cancelled and should be
reordered by your new primary care provider.

If you receive medications through patient assistance that are delivered to our office, please be
sure to contact your assistance program to change the delivery destination.

Your medical records will be kept confidential and will be transferred to another provider or
released to you or another person you designate only with your permission. After closing my
office, your health records from Lockhart Primary Care will be transferred to Morgan Records
Management, LLC (the “Records Custodian”). The Records Custodian will continue to maintain
your records in accordance with applicable confidentiality and security standards and with other
applicable laws. Your records will be destroyed no less than seven years after the last date of
services you received from Lockhart Primary Care. If you wish to have a copy of your records
sent to you or to another provider or facility, you may submit a written request to the Records
Custodian after May 1, 2025. Requests to the Records Custodian should be directed to:

Please Note:

• In accordance with applicable state law, you may be charged for the copying and transmittal of the records.

• There may be a transition period from the time I close to the time Morgan Records Management has access to my charts.
